Periorbital rejuvenation

Abstract

An area particularly susceptible to early signs of aging is the periorbital region, this process being influenced by both genetic and environmental factors and is characterized by the appearance of wrinkles, skin laxity, uneven texture, volume loss, dyschromia, dryness.

A multimodal approach incorporating both minimally invasive and non-surgical procedures, with minimal downtime and adverse effects can be used for periorbital rejuvenation, such as topical therapies, peelings, microneedling or mesotherapy, PRP, fillers, neuromodulation with botulinum toxin, laser and radiofrequency-based devices.

One of the most efficient modalities to treat hyperpigmentation and signs of skin aging of the infraorbital area, is the Q-switched Nd:Yag laser with a 1064 nm wavelength, which targets the deep dermis using nano seconds pulses that send acoustic shock waves causing collagen micro damage and a wound healing reaction, keeping the epidermis intact, with almost no recovery time and a high safety profile. When the pigmentation is too deep and significant, we usually use combined protocols with mesotherapy or injectables hyaluronic acid.

A hybrid CO2 and 1570-nm laser device, which allows treatment with 2 wavelengths simultaneous, both fractional ablative and fractional nonablative, can be used for a wide range of indications, with a short recovery period and a better side-effect profile. The fractional ablative laser can treat moderate to severe periorbital wrinkles and dermatochalasis by vaporization of the epidermis and part of the dermis, which leads to collagen production and skin tightening, at the same time the non-ablative fractional laser produces thermal injury in the dermis, while preserving the integrity of the dermis.

Although hyaluronic acid injectables remain a top choice today, there are many instances where improper use can worsen the condition. This is why it is essential to conduct a thorough assessment of the area and carefully select the appropriate combination of treatments, addressing all issues in the correct order to achieve optimal results. Periorbital rejuvenation continues to be an area of advancement especially in non-invasive or minimally invasive techniques.
