Body Dysmorphia Disorder: A Critical Role for Dermatologists
Abstract
Body dysmorphic disorder (BDD) represents a significant challenge within dermatological practice, as patients with this condition frequently present to dermatology clinics rather than psychiatric services. This systematic review of the literature (2005-2024) examined the current state of BDD management in adolescents, revealing critical implications for dermatologists' role in early detection and appropriate referral. The research identified that skin homogeneity constitutes one of the most common areas of concern among BDD sufferers, making dermatology clinics a primary point of contact. Due to insufficient insight into their condition and the delusional nature of their beliefs, patients with BDD rarely self-refer to psychiatric services, creating an essential gatekeeping role for dermatologists. The review found significant barriers including delayed help-seeking (up to 20 years), widespread misdiagnosis, and insufficient training of healthcare professionals in recognizing BDD. Six studies met inclusion criteria, with cognitive behavioural therapy identified as the most effective first-line treatment.
However, the literature revealed that screening in non-mental health settings, particularly dermatological and cosmetic surgery clinics, plays a crucial role in identifying BDD patients who would otherwise remain undiagnosed. Patients with delusional BDD beliefs demonstrate significantly elevated risk for seeking unnecessary cosmetic procedures, potentially resulting in patient harm and dissatisfaction with outcomes. The findings emphasize that dermatologists must be equipped with BDD screening tools and awareness training to fulfil their crucial position as first-line identifiers of this increasingly prevalent disorder. Integration of BDD education into dermatology training programmes and implementation of routine screening protocols in dermatology settings are essential steps toward improving patient outcomes and preventing inappropriate cosmetic interventions.
