Upper eyelid blepharoplasty: A case study of cosmetic outcomes following interrupted versus continuous subcuticular wound suturing
Abstract
Introduction: Blepharoplasty, or eyelid surgery, is an essential procedure in both aesthetic and functional plastic surgery, primarily aimed at correcting excess skin, muscle, and fat around the eyes. This surgery addresses age-related changes and eyelid malpositions that can impair vision and impact facial aesthetics. With a growing preference for minimally invasive techniques, blepharoplasty has evolved, incorporating advancements in surgical approaches and adjunct technologies that enhance outcomes and shorten recovery times.Two primary techniques are used for wound closure in blepharoplasty: the interrupted suture method and the continuous suture method.
Description: This study explores the critical stages of preoperative assessment, patient selection, and customized surgical planning to achieve optimal functional and cosmetic results in blepharoplasty. Following a detailed examination and skin marking, an incision was made in the skin of the upper eyelid, with muscle and fascia carefully separated. Three anchoring sutures were applied within the subcuticular tissue, securing it to the tarsal plate using 6-0 cutting proline sutures. In the study, wound closure was performed using two different techniques: an interrupted suture method for one patient group and continuous subcuticular sutures for the other. Suture were removed seven days postoperatively.Cosmetic results were visibly superior in patients whose wounds were closed with continuous subcuticular sutures, as they exhibited smoother scars. Conversely, a “train-track” appearance was observed in those with interrupted sutures. Some patients also experienced minor complications, including dry eye syndrome, lid malposition, and ptosis.
Conclusion: This session, grounded in case studies and real-world application, provides clinicians with the latest insights into blepharoplasty, focusing on techniques that enhance patient satisfaction and procedural safety.
Biography
Dr Gurumayum Chitralekha Devi is a dedicated and skilled dermatologist specializing in dermatologic surgery and cosmetic procedures. She completed her MBBS and internship in 2018 at Jawaharlal Nehru Institute of Medical Sciences (JNIMS). Following this, she pursued her MD in Dermatology, Venereology, and Leprosy from the prestigious All India Institute of Medical Sciences (AIIMS), Rishikesh, completing her residency in 2022. Since then, Dr Chitralekha has served as a Senior Resident in the Department of Dermatology at JNIMS, where she has developed extensive expertise in dermatosurgery, performing over 100 upper eyelid blepharoplasty procedures and also serving as secretary of IADVL(Indian Association of Dermatology, Venereology, & Leprosy) Manipur State Branch as well. Her research contributions include eight publications, reflecting her commitment to advancing dermatologic knowledge and patient care. Dr Chitralekha is highly interested in expanding her proficiency in dermatosurgery, focusing on innovative surgical techniques and improving cosmetic outcomes in her field.
