Efficacy of autologous patelet rich fibrin matrix in chronic non healing ulcer of hansen’s disease: a case series
Abstract
Chronic non healing ulcers are a frequent complication in patients with Hansen’s disease, primarily due to peripheral neuropathy, repeated unnoticed trauma, secondary infection, and compromised vascularity. These ulcers often persist despite standard wound care practices, leading to prolonged disability. Platelet rich fibrin matrix (PRFM), a second generation platelet concentrate has emerged as a promising biological dressing because it provides sustained release of growth factors and a fibrin scaffold that supports cellular migration and tissue regeneration. This case series assesses the efficacy of PRFM in promoting healing in non-healing ulcers among individuals with Hansen’s disease. Patients diagnosed with Hansen’s disease who presented with trophic ulcers persisting for >6 weeks were included. Following thorough wound bed preparation and debridement, autologous PRFM was prepared and applied directly to the ulcer surface. Repeat PRFM was done weekly changes in ulcer area, granulation tissue formation, pain reduction, and time to complete epithelization were monitored during follow up. 80% patents demonstrated notable improvement in wound healing following PRFM. 90% reduction in ulcer area and 92%% reduction in ulcer volume were evident within 2-3 weeks of treatment. No complications to PRFM were observed. PRFM appears to be an effective and well tolerated adjunctive therapy for chronic non healing ulcers in Hansen’s disease. Its regenerative properties may accelerate healing in ulcers refractory to standard care.

Dr Vejeta Rani has completed her MBBS, MD in Dermatology,Venereology & Leprosy, during which she was awarded a Gold medal for academic excellence. She has presented numerous scientific papers at state, regional, and national dermatology conferences. She is currently an Assistant Professor in the dept. of Dermatology at NSMCH, Jharkhand, India. Her role encompasses the management of a wide range of skin disorders, leprosy related disorders, underscoring her broad clinical competence.
