Evaluation of High-Fluence 1470 nm Non - Ablative Fractional Diode Laser in Cutaneous Scar Remodeling: A Two-Case Clinical Report and Quantitative Analysis

Abstract

Background: Cutaneous scars resulting from trauma or surgical intervention often cause functional impairment, cosmetic discomfort, and psychological distress. Non-ablative fractional lasers (NAFL) stimulate dermal neocollagenesis while maintaining epidermal integrity.

Objective: To evaluate the safety, clinical efficacy, and quantitative dimensional changes of a high energy 1470 nm NAFL protocol for scar management.

Methods: Two female patients with Fitzpatrick skin types II and III presented with facial scars following traumatic injury (Patient 1) and surgical lip-lift intervention (Patient 2). Treatments were administered using a 1470 nm diode laser (LaserMe) at high energy fluences (38–40 mJ/point) with tight spot spacing (1.0 mm). Dimensional metrics (length and width) and clinical scores via the Subject Global Aesthetic Improvement Scale (SGAIS) were monitored. Repeated Measures ANOVA was utilized to evaluate quantitative parameter reductions.

Results: Both patients exhibited statistically significant reductions in scar length (p<0.05) and scar width (p<0.01) across follow-up intervals. Skin texture smoothed considerably, with resolution of post-inflammatory erythema and alignment with surrounding cutaneous color. No persistent hyperpigmentation, scarring, or serious adverse events occurred during a 180-day follow-up.

Conclusion: High-fluence 1470 nm NAFL therapy offers a highly effective, safe, and low downtime treatment modality for facial scar remodeling, even in patients predisposed to melasma.
