Abstract

Background: Patients seeking facial aesthetic treatment frequently present with complaints such as a tired-looking gaze, dark circles, eyebrow descent and periocular wrinkles. However, the visible complaint may not represent the underlying cause of facial aging. Structural changes, soft-tissue support and muscular dynamics can contribute simultaneously to the appearance of the upper face. The Upper Frame™ concept was developed from clinical experience with facial assessment and hyaluronic acid-based rejuvenation, proposing an integrated approach to the facial frame surrounding the eyes.

Objective: To present the Upper Frame™ protocol as a structured and individualized strategy for rejuvenation of the upper third of the face, integrating anatomical assessment, muscular dynamics and structural support through the strategic association of botulinum toxin and hyaluronic acid.

Methods: The approach begins with a comprehensive evaluation of the upper third of the face at rest and during movement. The temporal region, eyebrow, periorbital area, orbital rim, tear trough and upper eyelid are assessed as interconnected anatomical components. Patient selection is based on the presence of structural and dynamic changes and on the limitations of non-surgical treatment. When indicated, botulinum toxin is used to address muscular dynamics and expression lines, while hyaluronic acid is considered for individualized structural support and volume restoration. The protocol follows a staged decision-making process, with reassessment according to the patient's clinical response rather than a rigid treatment sequence.

Clinical Contribution: The Upper Frame™ approach proposes a shift from treating isolated aesthetic complaints toward understanding the relationship between facial structure and muscular dynamics. Clinical cases involving patients with different degrees of upper-third aging illustrate the application of this strategy, including cases with temporal volume loss, eyebrow descent, infraorbital changes, periocular wrinkles and a tired-looking gaze. The approach also emphasizes appropriate patient selection and recognition of cases in which non-surgical treatment may have limitations or surgical assessment may be more appropriate.

Conclusion: Upper Frame™ presents a structured clinical framework for individualized upper facial and periocular rejuvenation. By integrating facial assessment, muscular management and structural treatment, the approach seeks to promote harmonious and natural-looking results while respecting anatomy, patient expectations and the limitations of minimally invasive procedures.
